Sign this waiver

Personally went to the dentists office today, calm, collected and well dressed. After getting more information about what the charges were and why i was quoted one amount and billed additional $301 dollars later, i asked for copies of everything they had in my file and figured that i'll just go over it myself and maybe find something that would help me dispute this better. On the 1R bus back to downtown as i looked through all the copies i found one paper which put this issue to rest. The official OFFICE FINANCIAL POLICIES waiver with my big fat signature on the bottom.

"IT IS THE PATIENTS RESPONSIBILITY TO KNOW THEIR BENEFITS, YEARLY MAXIMUMS AND REMAINING BENEFITS. WE DO NOT CALL YOUR INSURANCE COMPANY UNLESS YOU REQUEST US TO DO SO. EVEN THEN IT IS NOT A GUARANTEE OF PAYMENT."

Ok, i'm fine with that. But what a bout the $280 i was quoted which should have actually been $581?

Patient co-payments are vary with each insurance policy and are quoted as accurately as possible and are an ESTIMATE ONLY.

Well that it. The dispute killed with one waiver.
